Course requirements:

All students must show documentation that they are eligible to take a driver education course.

Northwest Suburban Driving School will only require 3 items in order to file for a certificate of completion.

These 3 items include:  Click below to obtain our forms !

1. A copy of the student's permit,
2. A signed blue form or copies of the student's last 2 semesters of report cards.
3. A signed completion of classroom form.

Students must provide a copy of their most recent report card, which shows the grades from the last 2 semesters of classes. If a student cannot supply a copy of their report card, Northwest Suburban Driving School can provide a blue form which can be completed by an administrator from their past or present school.

Students must also provide a letter from their school which lists the date of classroom completion.

Northwest Suburban Driving School can also provide all of the necessary information and application forms, so students can receive their instructional permits.
